A Team Approach To Improve Quality of life

The Canadian healthcare system is in the process of changing. The old way healthcare delivery system clearly is not working. One approach to meet the high level of care for patients and their families is to use all the resources available to design better care for patients. In order to improve patients ' quality of life, it is important to focus on effective and efficient care to patients with chronic health issues. The use of a multidisciplinary team approach is on the way to provide better care when compared to the old healthcare system (individual patient- physician care). The team care approach involves many other health professionals such as physicians, registered nurses, nurse assistance, dieticians... The patient centred care shows positive measurable outcomes and therefore can improved quality of life; reduce rehospitalisation and prevent other complications.
